• Mobile concierge visits are available upon request and pending availability.

Fees

• A $20 mobile service fee applies for locations within 20 miles of our office.
   

• Additional fees may apply for visits outside this radius.
   

• The mobile service fee is waived if the total service cost exceeds $200.

Booking Requirements

• All intake forms and consents must be completed prior to the scheduled appointment to allow adequate review by our medical team.
   

• A credit card must be placed on file at the time of booking, but it will not be charged until the appointment time.
   

Cancellation Policy

• Appointments not cancelled within 48 hours of the scheduled appointment time will incur a $50 fee.
